When do hero dice refresh?
Say I have a scenario that runs over two session, do they get 1 HD for the whole scenario or does it refresh at the start of each session.
Same question with regards to never surrender that can be used each issue?

I believe the official RAW is that a player gets 1 Hero Die (= 5 Hero Points) each "issue". Issue defined as 1 game session.
That would be on TOP of any additional Hero Points (or Setback points) each PC individually gets for their Character Point build total compared to the Narrator's "Campaign Level" for the Story Arc. Meaning... As the Narrator I look at the treats, etc... I have planned for this entire adventure (multiple issues) and say it's at 34pts. (example). Then each PC-Hero compares their build point total to that. If you're under 34pts you GET "x" amount of Hero Points. If you're over 34pts, the Narrator GETS "x" Personal Setback Points to use against your PC-Hero.
I know BASHMAN (unofficially from RAW) suggests always setting each Issue's "level" to 5 pts higher than the highest Character Point built PC-Hero - thus automatically giving EACH PC-Hero 2 starting Hero Dice (= 10 Hero Points) on top of any additional for each PC-Hero's Character Point build vs. the Issue Level.
To be honest.... the BASH! Rules As Written for this, for the Narrators are very very sketchy IMHO. Not well defined, and the "keywords" are not well set - to help a Narrator consider all the above.
And I don't think at any place does the RAW suggest how/when to "carry over" anything.
So...
What I do (which would therefor be a House Rule) is:
- I like to set my "Issue" level: this is for each game session, because this issue of the entire Adventure might be an opening adventure with minor minion threats (that might only be 27 pts) - compared to next Issue which might be a major encounter with the Big Bad (that might be 38 pts!!)
- I announce the Issue Level at the beginning of each game session. The PC-Heroes compare and either ask for their extra Hero Points or tell me (as Narrator) to take their Personal Setback Points.
- I then give each PC-Hero their "FREE" 1 Hero Dice for the Issue (single game session).
- I then ask if anyone has the ADV: Leadership or anything else that gives them additional HP or HD. This includes other Personal Setback Points to be taken by the Narrator.
- During the game session PC-Heroes will gain additional HPs (or Setback Points!) as they do things worthy...
- At the END of the game session (Issue), if still in the same "Adventure / Story Arc", I allow them to "carry over" ANY HPs and HDs, and I (as Narrator) "carry over" their Personal Setback Points.
- I DONOT allow them to carry over individual Leadership Points. (though, to simplify, there's really no reason I shouldn't)...
The KEY point to remember is... that by the RAW. You can decide to give each PC-Hero their 1 FREE Hero Die at the begining of EACH Issue (single session) or EACH Adventure (multiple sessions). As written, I believe the rules imply this if so each Adventure - not each Issue.

Not exactly. I set it to 5 beyond the "cap" for the game. That is a bit different. I also adjust the point count for each issue I run. So if they are facing a much more challenging opposition, I might set the game at 40 points for that issue.
But to answer the OP's question, yes, they refresh every issue. If you are playing for an especially long stretch, you may even divide that day's game into more than one issue (I tend to do this at conventions when I am running an 8-hour session, dividing it into two 4-hour issues).
